The treatment of children with non-Hodgkin’s lymphoma is less well defined than in adults. To evaluate the outcome of children with newly diagnosed non-Hodgkin’s lymphoma, with special reference to the development of new, potentially curable forms of the disease and the reirradiation of residual disease. A retrospective survey of the outcome of 90 children with non-Hodgkin’s lymphoma diagnosed at the Royal Marsden Hospital between 1974 and 1982 was conducted. The overall 5-year survival was 56% with a median follow-up of 9 years. A new clinicopathological entity, anaplastic lymphoma arising within low-grade follicular lymphoma, was diagnosed. At presentation all children had low-grade tumours but 15% developed intermediate-grade or high-grade disease and were treated with chemotherapy and irradiation similar to the protocol developed at the Royal Marsden. Among the 41 children with low-grade disease, 22 had low-grade lymphoma alone; none of these relapsed or died. Among the children with intermediate-grade or high-grade disease, 26 (63%) had B-cell disease alone, and no children developed a new pathology. Among these, 26 children received non-immunochemotherapy, and none of the intermediate-grade disease nor 3 (13%) of the high-grade disease relapsed or progressed. Three of the 26 children with intermediate-grade or high-grade disease were reirradiated with a good response. Children with low-grade lymphoma, irrespective of whether they have low-grade lymphoma alone or whether a new pathology arises,
